Ring1: Ring of Entropy from AQ
Ring2: Healing one from domo
Trinket1:Lifegiving gem
Trinket2: Hibernation Crystal(Clickable healing item from Green Dragons)
Weapon: Widow's Embrace Hammer from Faerline in Naxx
Shields: Both the Rag and Vael shields
Mount: LEGENDARY MOUNT FROM OPENING AQ

As for pvp gear this guy has full tier 2 and hammer of woe. I have various tanking gear pieces from AQ and some of the other pieces from BWL. I have awesome resist gear for every single resist needed and this character has Frozen Runes on him too for making his frost resist gear and the other items needed to make it.
