My provider of choice only lets your scripts connect from 'localhost', so I'm out on that option.
I need to be able to connect to a MySQL with very, very fast response time. Approx 20GB / month of bandwidth will be used.
Willing to pay as much for this as regular web hosting.

if you run out of options......
I use 1and1.com
SUPER fast and has like up to 50G/month XFER for like umm 39.99 (actually I think you get your own box with root for this price)
or free for 6 months of 10G / month or such.
has mysql/php/linux
