r00tus will host your site for 4 dollars a month, as an added bonus he'll give you all that *and* more! Well... maybe not 500 email accounts...
edit: seriously though, r00tus is desperate to get paying customers, talk to him =p

only problem so far is they wont update the GD lib for me. wont install magick either so you cant get convert. this is on the what was free which is now $5/month plan.
500GBs, 5GB xfer/month, ssh, ftp, web admin, php, mysql, apache

i will say that, i've been kicked off numourous cheep webhosting plans when i started using up my 'allotment' of bandwidth.
if you really want to get offers on webhosting without searching, go to http://www.webhostingtalk.com/ and post in their request forums. tell them exactly what you want and you'll get more quotes then you'll ever want ot look at
