How is this done, exactly? I assume there's a toolset of some sort, but is it intuitive enough for new users to pick up along the way? Or is there a learning period – how long would that be, on average? Optionally (though not likely), I suppose SL could be using some 3rd-party modeling tool that the users are expected to be familiar enough with? Is that the case, perhaps?

Most of the information you're after here is covered in the official FAQs and help files. In brief, Second Life does have its own set of building tools, and yes, a certain amount of time is required to adjust to them, especially if you've never done any visual modeling before.
